As a Care Coordination Manager at Frontera Health, you will play a vital role in ensuring that our clients receive timely and necessary care by managing our care coordination and scheduling functions. This role combines overseeing active and waitlist care coordination and scheduling across our Applied Behavior Analysis (ABA) therapy Clinics and Frontera Health Diagnostic services. The Care Coordination Manager role is essential for ensuring exceptional customer service for our clients and their families.
Key Responsibilities:
- Care Coordination Management
- Lead and oversee care coordination for Clients from referral through graduation of ABA therapy, ensuring smooth and timely service delivery
- Manage the Client pipeline to ensure timely movement through our waitlist and create a positive experience through timely and personalized communication with our waitlist Clients
- Monitor and measure adherence to internal processes, compliance policies, pipeline throughput, and customer experience
- Schedule Oversight
- Manage the scheduling functions for internal clinical employees and external clinical contractors including our:
- Psychologists for Comprehensive Diagnostic Evaluations scheduling related to our waitlist and active Clients
- Initial Assessment BCBA scheduling to ensure timely initial assessment for our waitlist Clients
- BCBA and RBT scheduling to create and maintain therapy schedules for our active Clients
- Coordinate with Clients to match availability to our internal employee and external contractors for Evaluation/Assessment appointments and therapy schedules
- Measure and escalate adherence, for Employee and Clients, to appointment and therapy schedules
- Partner with Scheduling Coordinators to troubleshoot scheduling conflicts, reschedule sessions as needed, and proactively communicate changes with clients and staff
- Manage the scheduling functions for internal clinical employees and external clinical contractors including our:
- Process Improvement
- Collaborate with team members to identify and analyze workflows for care coordination, optimizing efficiency and reducing errors
- Propose and implement process improvements to achieve faster turnaround times and improve service delivery
- Utilize data analysis and best practices to refine care coordination and scheduling processes
- Team Management
- Lead a high-performing care coordination team responsible for supporting our Clients and Clinical team
- Create clear communication and cadences to provide transparent updates about our pipeline and scheduling performance to Leadership team
- Facilitate team meetings, as necessary, to enable team collaboration and communication
- Provide mentorship, coaching, and development opportunities for team members
Qualifications:
- Bachelor’s degree in healthcare administration, business, or a related field preferred
- Experience in care coordination, scheduling, or logistics preferred
- Two to three years of experience leading a team of two or more direct reports
- Strong analytical and problem-solving skills with a passion for process improvement
- Excellent communication and interpersonal skills with the ability to build rapport with clients, families, and colleagues
- Proficiency in Google Suite, healthcare EMR systems, and data analysis tools
- Demonstrated commitment to providing exceptional customer service and exceeding client expectations
- Adherence to HIPAA regulations and patient confidentiality standards.

We have determined a salary range for this position that takes into account several factors including experience, knowledge, education, skills, and abilities. Please note that the salary information is a general guideline and the exact salary will be determined based on the individual’s qualifications and experience, with consideration given to the factors listed above.
Expected salary range in Denver:
$110,000-$130,000
